Eligibility
All current and new patients will need to enroll in Virginia's
​New Medicaid Health Insurance Program.  
Please see the Medicaid page for detailed information. ​

We treat patients as individuals with dignity and respect

If you are a resident of Fauquier or Rappahannock counties who meets our household income requirements, you are eligible for free medical, mental health and dental services. Patients with no health insurance and those enrolled in Virginia's Medicaid health insurance program are welcome at the Free Clinic.

Eligibility Screening

All patients are screened to determine their eligibility for services at the Free Clinic. In order to determine if you are eligible we will ask you to produce certain documents so we can verify your residency, calculate your income and confirm any Medicaid coverage if you have that.  Our screeners want to make sure that everyone who qualifies for our services is granted eligibility and will work with you on a case by case basis to determine your eligibility. 

If you are currently a patient or have been seen at the clinic within the past year, you can update your eligibility before an appointment or during office hours when a screener is available to help you. You may also drop off or mail documents so we can update your eligibility.  A PDF of a document may be emailed to info@fauquierfreeclinic.org


Only patients who are currently eligible will receive services, including medication refills.

It is to your advantage to know when your eligibility expires and make every effort to keep it current.  

What to bring

Proof of Residency
Examples are a valid Driver's license, ID card, current lease, vehicle registration or a bill dated within one month.  Your proof of residency must have your name and your PHYSICAL ADDRESS on it.  We cannot accept documents with PO Boxes on them as proof of your residency. 
If you are working
One month of pay stubs or bank statements showing direct deposits. If you are working for cash we will give you a Verification of Employment form to be filled out by your employer or click the button below to download and print.

If you file taxes
Your most recent 1040 or 1040 EZ return. We do not need your W-2.  If you are self employed we will also need a copy of your Schedule C.
Other types of income
If you receive Social Security, Disability, Veterans Benefits, pensions, SNAP benefits, Unemployment Compensation or child support we need a statement/court order from the provider covering the current period.
If you have no income
If you have no visible means of support, we will give you a form to be filled out by the individual supporting you or click below to download and bring with you.
