Patient Eligibility

We treat patients as individuals with dignity and respect.

If you are a resident of Fauquier or Rappahannock counties who meets our household income requirements, you are eligible for free medical, mental health and dental services. Patients with no health insurance as well as those enrolled in Virginia's Medicaid health insurance programs are welcome at the Free Clinic.

Eligibility Screening

All patients are screened annually, in advance of their Eligibility expiration date, to determine their eligibility for services at the Free Clinic.

Screening takes place by appointment on the phone with a remote screener and, if required, a translator.  Only patients who are eligible can receive services, including medication refills, so it is important to schedule screenings at convenient times and to keep the appointment. Any delay in screening may result in the cancellation of upcoming appointments until you can be re-screened.  

To prepare for your screening appointment, please have all household income information on hand as well as Medicaid cards if applicable. In order to determine if you are eligible we will ask you to provide documents so we can verify your residency, calculate your income and confirm any Medicaid coverage if you have that.  Our screeners want to make sure that everyone who qualifies for our services is granted eligibility and will work with you on a case by case basis to determine your eligibility. 

If you are currently a patient or have been seen at the clinic within the past year, you can update your eligibility before an appointment or during office hours when a screener is available to help you. You may also drop off or mail documents so we can update your eligibility.  A PDF of a document may be emailed to

Only patients who are currently eligible will receive services, including medication refills.

It is to your advantage to know when your eligibility expires and make every effort to keep it current.  

What to bring

Proof of Local Residency
Your Proof of Address (POA) must be dated within 3 months of your screening and include your Name and current physical address.  Examples are a Valid Driver’s license, DMV ID, utility bill, bank statement, annual lease or an official piece of mail. PO Boxes are accepted on a case-by-case basis.  Junk mail, credit card solicitations, voter or vehicle registration cards and handwritten letters are not accepted.
If you are working
One month of pay stubs or bank statements showing deposits. If you are working for cash we will give you a Verification of Employment form to be filled out by your employer.  You can also print the form from the "Patient Forms" page. 
If you file taxes
Your most recent 1040 or 1040 EZ return. We do not need your W-2.  If you are self employed we will also need a copy of your Schedule C.
Other types of income
If you receive Social Security, Disability, Veterans Benefits, pensions, SNAP benefits, Unemployment Compensation or child support we need a current statement or a copy of a court order.
If you have no income
If you have no personal means of support, we will give you a simple "Letter of Support" form to be filled out by the individual supporting you. You can also print the form from the "Patient Forms" page. ​
Once your documents have been received and reviewed by the Eligibility Coordinator, you will be notified of your eligibility status. Until then, you will not be eligible for any clinic services, including medication refills, so please provide your documents as soon as possible.
